POSITION SUMMARY:
The associate shall be responsible for providing a full range of speech-language pathology services including assessment, treatment planning and therapeutic interventions in a team environment consistent with the position’s qualifications, professional practices, and ethical standards. The associate shall also demonstrate accountability for and contribution to program development, quality improvement, problem solving and productivity enhancement in a flexible team fashion. The Speech Pathologist reports to the Clinical Director.
ESSENTIAL JOB FUNCTIONS:
· Ensuring a safe and therapeutic environment.
· Ability to practice as an effective member of an interdisciplinary team.
· Able to speak, write, read, and comprehend English.
· Conduct initial assessments, develop individualized treatment plans and provide age-appropriate therapeutic interventions.
· Evaluate patient response to treatment and document the course of patient care in the medical record including progress made and continuing need for treatment.
· Speech-Language Pathologists (SLP) evaluate and provide skilled speech-language therapy to individuals in a home setting, under the care of an attending physician. At Integrity Hearts Health Services, the home setting may include single or multi-family homes, assisted living, independent living, or memory care.
· The SLP evaluates and treats patients with language impairments, motor speech disorders, dysphagia, cognitive communication difficulties, voice disorders, and those requiring alternative or augmentative communication.
· Make home visits to clients in designated geographic territories.
· Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients who are experiencing speech, language, communication, or swallowing difficulties.
· Develop a treatment plan based on the evaluation, physician’s orders, and recommended goals.
· Continually assess and revise the speech therapy care plan, and participate with the multidisciplinary care plan, as appropriate.
· Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
· Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
· Ability to travel to cases, as assigned
EDUCATION:
· Graduate of a master’s/bachelor level program in speech-language pathology
· A minimum of one year of clinical experience is preferred
LICENSING & CERTIFICATION:
· Current speech pathologist licensure in good standing in state where services are performed.
· Basic Life Support (BLS) – American Heart Association
· Possess a valid state driver’s license or valid identification card and automobile liability insurance if applicable
