POSITION SUMMARY:
The home health Registered Nurse will provide direct skilled patient care, observation, and educational instructions in the place of residence under the direction of the patient's certifying physician. Home health nursing focus is on health promotion and care of the sick while integrating environmental, psychosocial, economic, cultural, and personal health factors affecting an individual's and family's health status. The home health registered nurse will apply nursing practice to patients of all ages in the patient's residences, which may include private homes, assisted living, or personal care facilities. Home health nursing embraces primary, secondary, and tertiary prevention; assistance to family with coordination of community resources and health insurance benefits; and delivery of healthcare services in a patient's home, including non-conventional residences. The home health registered nurse plays a key role in the financial management of the patient's episode of care. This includes verification of patient insurance and the management of the HRRG reimbursement for traditional Medicare eligible home health clients. The Registered Nurse reports to the Clinical Director.
ESSENTIAL JOB FUNCTIONS:
· Follow a designated care plan in accordance with patient's needs.
· Make home visits to clients in designated geographic territories.
· Performing assigned duties, including administration of medication, wound care, treatments, and procedures in compliance with the State of Practice and Integrity Hearts Health Services policies and procedures.
· Monitoring clients' conditions; reporting changes to Clinical Nursing Director.
· Follow up with, execute, and properly document doctors' orders.
· Perform client assessments as necessary.
· Case management and coordination.
· Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
· In addition to performing visits and completing coordination of client care, the Registered Nurse (RN) Case Manager is the point of contact for all disciplines involved with providing care to patients and oversees the frequency of visits for the episode.
· The RN Case Manager consults as needed with the physician and the office giving details about patient care.
· Performing accurate OASIS collection, ensuring the medication profile remains current
· Ensuring a safe and therapeutic environment.
· Conduct initial and ongoing patient assessment.
· Evaluates, updates and revises plan of care to facilitate achievement of planned and expected outcomes.
· Implementing clinical nursing interventions.
· Administer medications in a safe manner consistent with the State of Practice and Integrity Hearts policies and procedures.
· Able to speak, write, read, and comprehend English.
· Ability to travel to cases, as assigned.
EDUCATION:
· Graduate of an accredited Associates degree or Bachelor’s degree in nursing
· A minimum of one year of clinical experience is preferred
LICENSING & CERTIFICATION:
· Current registered nurse licensure in good standing in state where services are performed.
· Basic Life Support (BLS) – American Heart Association
· Advanced Cardiac Life Support (ACLS) – American Heart Association or Red Cross
· Possess a valid state driver’s license or valid identification card and automobile liability insurance if applicable
