POSITION SUMMARY:
The associate shall be responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in a team environment consistent with the position’s qualifications, professional practices, and ethical standards.
The associate shall also demonstrate accountability for and contribution to program development, quality improvement, problem solving and productivity enhancement in a flexible team fashion. The Physical Therapist reports to the Clinical Director.
ESSENTIAL JOB FUNCTIONS:
· Ensuring a safe and therapeutic environment.
· Make home visits to clients in designated geographic territories.
· Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
· Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
· Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
· Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
· Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.
· Ability to practice as an effective member of an interdisciplinary team.
· Able to speak, write, read, and comprehend English.
· Ability to travel to cases, as assigned.
EDUCATION:
· Graduate of Physical Therapy curriculum recognized by the American Physical Therapy Association
· A minimum of one year of clinical experience is preferred
LICENSING & CERTIFICATION:
· Current physical therapy licensure in good standing in state where services are performed.
· Basic Life Support (BLS) – American Heart Association
· Possess a valid state driver’s license or valid identification card and automobile liability insurance if applicable.
