POSITION SUMMARY:
Employees are to deliver patient-centered care and service delivery. Each Employee must comply with the client’s care plan that addresses each client's preferences, daily needs, interests, activities, and personal goals.
Personal Care Services provides support to people with disabilities of all ages, elderly (over 65 years) and people with chronic health conditions with activities of daily living, instrumental activities of daily living and observation and redirection of behaviors. Any person providing services under personal care services reports to the Clinical Director.
ESSENTIAL JOB FUNCTIONS:
· Ensuring a safe and therapeutic environment
· Monitoring clients' conditions; reporting changes to Clinical Nursing Director
· Answer client questions about self-care techniques and give instructions that will assist family, client, and family/support in providing care.
· Perform light homemaking assistance, including help with personal laundry, meal preparation, and food shopping, running errands
· Making home visits to clients living in designated territories
· Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing medical records
· Able to speak, write, read, and comprehend English
· Take and record vital sign checks: temperature, pulse, respiration, blood pressure, oxygen(SPO2)
· Accompany clients to recreational/social events and/or appointments.
· Participate on the Care Team by providing input and making suggestions.
· Ensure service is delivered in accordance with Integrity Hearts Health Services policies, procedures, and industry standards.
· Monitor supplies and resources.
· Carry out assigned tasks on clients and follow Home Care Plans.
· Transfers /use of mechanical lifting devices
· Activities of daily living including: bathing, dressing, toileting, feeding, range of motion/exercises, ambulation, etc.
· Patient data collection and documentation (i.e. vital signs, intake/output, change in patient condition)
· Provides accurate, precise, timely documentation when applicable for patient care
· Ability to travel to cases, as assigned

· Respite Care Services
EDUCATION:
· High School Diploma or GED Equivalent
· A minimum of one year of clinical experience is preferred
LICENSING & CERTIFICATION:
· Current personal care aide certification in good standing in state where services are performed
· Basic Life Support (BLS) – American Heart Association
· Possess a valid state driver’s license or valid identification card and automobile liability insurance if applicable
