POSITION SUMMARY:
Under the direction of a Licensed Professional (e.g., Physician or Nurse Practitioner), the Licensed Practical Nurse (LPN) is responsible for the delivery of patient care utilizing the nursing process through data collection and assessment, identifying, and determining the priority of patient’s problems/needs, and evaluating the process and outcome of nursing care. The LPN coordinates, provides, and directs patient teaching activities and coordinates the care provided by health team members. The LPN complies to all documentation standards within the scope of the LPN role. The Licensed Practical Nurse reports to the Clinical Director.
ESSENTIAL JOB FUNCTIONS:
· Ensuring a safe and therapeutic environment.
· Making home visits to clients living in designated territories.
· Performing assigned duties, including administration of medication, wound care, treatments, and procedures in compliance with the State of Practice and Integrity Hearts Health Services policies and procedures.
· Monitoring clients' conditions; reporting changes to Clinical Nursing Director.
· Following up with, executing, and properly documenting doctors' orders.
· Performing client assessments as necessary.
· Case management and coordination.
· Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing medical records.
· Ability to practice as an effective member of an interdisciplinary team.
· Implementing clinical nursing interventions.
· Good Driving record with the ability to drive in inclement weather and at night.
· Coordinates the care of the client according to the client's plan of care.
· Able to speak, write, read, and comprehend English.
· Current and valid driver's license with acceptable driving record, ability to provide own dependable transportation for home visits and proof of auto liability insurance.
· Ability to travel to cases, as assigned.
EDUCATION:
· Graduate of an accredited Licensed Practical Nurse diploma, certificate, or associate
· A minimum of one year of clinical experience is preferred
LICENSING/CERTIFICATION:
· Current Licensed Practical Nurse (LPN) in good standing in state where services are performed
· Basic Life Support (BLS) – American Heart Association
· Possess a valid state driver’s license or valid identification card and automobile liability insurance if applicable
